well-defined thin (≤2 mm) smooth wall; homogeneous simple fluid (-9 to 20 HU) no septa or calcification; the wall may enhance after administration of contrast; Class II . six types all with thin The Bosniak Classification, version 2019 (a) formally incorporates MRI into the classification, (b) includes specific definitions for individual imaging features and Bosniak classes, (c) incorporates a larger proportion of renal masses encountered in clinical practice (eg, incompletely characterized but highly likely benign cysts), and (d Percentage of final reports for imaging studies for patients aged 18 years and older with one or more of the following noted incidentally with a specific recommendation for no follow‐up imaging recommended based on radiological findings: Cystic renal lesion that is simple appearing* (Bosniak I or II) Adrenal lesion less than or equal to 1.0 cm Adrenal lesion greater than 1.0 cm but less than This CT-based classification was introduced in 1986 and originally divided cystic renal masses into one of four classes after exclusion of infectious, inflammatory, and vascular etiologies ( Table 1) (1). Since then, refinements have reduced the number of benign masses in Bosniak class III (2–9). For example, Bosniak IIF (where the F The Bosniak classification categorizes renal cysts into five groups. Category I. Benign simple cyst with thin wall without septa, calcifications, or solid components, and has a density of 0–20 Hounsfield units (HU) (about equal to that of water). In such cases, a CT scan without intravenous contrast is enough for classification.

A total of 3,036 complex renal cysts were categorized into Bosniak II, IIF, III and IV. In surgical and radiological cohorts pooled estimates showed a malignancy prevalence of 0.51 (0.44, 0.58) in Bosniak III and 0.89 (0.83, 0.92) in Bosniak IV cysts, respectively. Bosniak classification version 2019 proposed refinements for cystic renal mass characterization and now formally incorporates MRI, which may improve concordance with CT. OBJECTIVE. The purpose of this study is to compare concordance of CT and MRI in evaluation of cystic renal masses using Bosniak classification version 2019. The Bosniak classification in these cases may even be unnecessary, as cystic lesion with a solid component will always be removed whereas a simple cyst will be followed. A subsequent study by Karmazyn et al. 13 reported moderate interobserver agreement (kappa =0.48), which improved (kappa = 0.65) when the population was divided into binary classes. This Bosniak group would remain active for several decades, with the continuity of ideas and the use of the Bosniak name.
